ATHLETIC FORMS

If you have lost your athletic forms packet, please contact the office at (952) 556-0040 and one will be sent to you or given to your student.  There is also a copy of the forms available on the News and Notes page of the website (scroll to bottom of page).  Please keep in mind that the forms packet must be turned in prior to the first practice in order to participate in athletics.  The $310 sports fee is due at the end of the first week of practice.

Athletic physicals are good for three calendar years from the date of the physical.  Once the physical has expired, another must be conducted with a form on file in the athletic office.  A downloadable form is available on the right.  If you have any questions regarding the expiration date of your student's physical, please contact Jon Longnecker in the athletic office at (952) 556-0040 x109.

Form explanations:

Sports Qualifying Physical Exam – Schedule a physical with the family doctor (preferably as soon as possible) and have him or her fill out this form. This form remains on file for three years. After three years, you must have this done again. *Many people get their physical as a freshman, and, as a result, need another physical prior to their senior year.
